Job Description

Position Summary


The Per Visit Certified Occupational Therapy Assistant (COTA), under the direction of the Rehab Director/Manager provides therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient's plan of care within the homecare setting. Provides therapeutic treatment and instruction to patients to improve/restore, reinforce and enhance task performances, diminish or correct pathology and promote/maintain self-sufficiency. Other responsibilities will be assigned as appropriate.

Minimum Requirements



• Education Graduate of an Occupational Therapy Assistant program accredited by the Accreditation 

Council for Occupational Therapy Education, (ACOTE) of the American Occupational 

Therapy Association, Inc. or its successor organizations

• Experience One (1) year of clinical experience.



Required Licenses/Registrations/Certifications


• Current license to practice as an Occupational Therapist in South Carolina

• Must maintain National Certification for OT 

• Valid SC Driver's License with good driving record


Core Job Responsibilities


• Carries out tasks of the treatment plan as outlined and reviewed with the Occupational Therapist, and within the professional scope of practice and individual level of expertise. 

• Assists in providing rehabilitation services for patients who have difficulty performing work and daily living activities due to illnesses, injuries and disabilities.

• Assists patients in performing exercises as outlined by the Occupational Therapist and teaches them safe and effective equipment use.

• Teaches patient and family/ caregivers in meeting individual treatment goals, and treatment plan implementation as outlined by the Occupational Therapist. 

• Notifies the supervising Occupational Therapist of changes in patient status, function, or medical condition 

• Prepares/completes clinical notes that comply with all state and federal mandates.
